Waivers

In order to meet with our staff we require the following: Privacy Policy, Confidentiality and Consent to Serve waivers.

If your student is under 18, please use the following forms and we require both parent and participant signatures:

If your student is over 18, please use the following forms and we require both parent and participant signatures:
